Credit refund conditions

• Only applicable to payments of Muang Thai Life Assurance PCL's premiums using Muang Thai Smile Credit Card
  
• Premiums for single-premium insurance, short-term insurance, protection-only insurance, and all programs of personal accident insurance (except PA return bonus) are not eligible for refund credit
• Premium payment by all types of installment payment, debt payment to the policy loan and interest, debt payment to an automatic premium loan (APL) and interest, and premium payment with a credit card's reward points are not eligible for refund credit
• Kasikorn Bank will credit the refund amount to the credit card within 3 accounting cycles from the monthly balance concluding date and after full payment is made according to the specified conditions
• The calculated refund credit amount to the credit card will be displayed and used to deduct credit card spending on the card's statement for the month crediting the refund
• The refund credit amount to the credit card cannot be converted to cash or transferred to other persons
• There is no limit in using the credit card to pay for minimum insurance premiums to be eligible to credit the refund amount to credit card
• Cases of premium refund transactions, and all cases of cancelling or rejecting payment for transactions of credit card usage to pay premiums, are not eligible for refund credit. If such cases occur after Kasikorn Bank has already refunded the amount into the credit card account and deducted the spending, Kasikorn Bank shall collect the refund credit from that credit card account
• Reserved only for credit card holders whose spending meets the conditions and who retain membership of the credit card with a credit card payment history meeting Kasikorn Bank's conditions
  

Conditions for Getting 2 fold Reward Points

• The premium needs to be paid through Muang Thai Smile Credit Card to Muang Thai Life Assurance only
• Conditions to receive KBank Reward Points are under the regulations set by the Bank
• Conditions to receive Smile Points are under the regulations set by Muang Thai life Assurance
  

Conditions for Getting Special KBank Reward Points

• Get 3 times special reward points only for Pink Gold cardholders and 2 times only for Prestige, Platinum, and Smile cardholders by spending in shops designated by Kasikorn Bank
• Special reward points are reserved for shops designated by Kasikorn Bank. Points are calculated based on merchant category code (MCC): 5940 (bicycle shops), 5941 (sporting goods stores), 7298 (health & beauty spas), 7941 (commercial and professional sports clubs, athletic fields, and sport promoters), 7997 (membership clubs), 7999 (recreation services), and specified merchant category codes when collecting to Kasikorn Bank
• Maximum special reward points are limited to 10,000 points in each credit card cycle
• Kasikorn Bank shall transfer the special reward points to cardholders whose spending meets the conditions, who retain membership of the credit card, and whose credit card payment history meets Kasikorn Bank's conditions, within 2 accounting cycles after the cycle with the spending that meets the conditions
• Reward points cannot be exchanged or converted to cash

This life insurance will also protect your spouse and children below 23 years of age whose fare is also paid through Muang Thai Smile Credit Card.

Service Scope of Muang Thai Smile Secretary

Muang Thai Smile Secretary is at your service for 24/7

1. Personal secretary service….Muang Thai Smile Secretary will help coordinate with and search for service provider, and give practical information only. However, any incurred expenses from services chosen by customers, the customers will be fully responsible for it.
2. Muang Thai Smile Secretary service is FREE for Prestige and Platinum Muang Thai Smile Club members. Simply receive this privilege by registering at Tel. 1766, press 5 without using accumulated Smile Points (Muang Thai Smile Secretary service shall end once the status of Prestige and Platinum or policy ends.).
3. Muang Thai Smile Club members (who are not Prestige and Platinum members) are entitled to redeem 30 accumulated Smile points for Muang Thai Smile Secretary service which is valid for 1 year (redemption period is from today – 30 December 2016, Tel. 1766, press 4.).
4. Personal secretary service…Muang Thai Smile Secretary shall be invalid once life insurance policy ends. The Company reserves the right to change service provisions as deemed appropriate without prior notice.

Birthday gift for Pink Gold or Prestige Muang Thai Smile Club members:

• Only for members whose birthday is during November 2016 - October 2017.
• Customers will receive the gift within the birth month.
• This is reserved for customers with Pink Gold or Prestige status and during birthday only.

Smile Health Checkup Voucher

(for Pink Gold or Prestige according to the original conditions before 1 January 2014)

Issue Conditions of Health Checkup Voucher (according to the original conditions)

1. The customers must be Pink Gold or Prestige Smile Club members.
2. Each year, the customers shall receive health checkup voucher only when the customers pay renewal premium within 31 days counting from payment due date of renewal premium of year 2 onwards
3. Health checkup voucher of the following years will be issued only when the customers maintain status as stated in the condition No. 1 and No. 2. The voucher will be issued 12 months after the previous issue.

Validity Period of Health Checkup Voucher:

The voucher is valid for 6 months after the issued date in which the customers can make an appointment with participating hospitals in the Smile Health Campaign to receive the service as specified in the voucher(the time period is specified in the health checkup voucher such as issued date in 01/14 will be valid until 06/14. This means the validity period is from 1/1/14 to 30/6/14 only.)

*Remarks: Health checkup voucher will be issued 30 days after the date that the Company receives premium payment within the payment due date of renewal premium of year 2 onwards.

Privileges of Health Checkup Voucher

1. To be used for health checkup according to Smile Health checkup program based on age range with free of charge!
2. The voucher includes health checkup services for 2 guests together with the voucher holder which they can choose different health checkup programs with special price (for more information, please contact a servicing hospital). However, the holder and guests must take health checkup on the same date, time and location.
3. The voucher must be used within expiry date specified in the health checkup voucher.

Conditions of Accumulated Muang Thai Smile Point Redemption

1. Muang Thai Smile Club members are entitled to redeem accumulated points for specified health checkup programs by informing your chosen program and hospital to a staff on the accumulated points redemption date.
2. Muang Thai Smile Club members can contact to redeem accumulated points at Tel. 1766, press 4, Muang Thai Smile, during office hours on Monday – Friday from 8.30 – 17.00 hrs.
3. Each member is entitled to redeem the main health checkup program (up to 1-2 programs only) and supplementary programs (can select more than 1 program).
4. The health checkup program which has already been redeemed cannot be changed, transferred to other persons, or cancelled to retrieve the accumulated points.
5. Details of point redemption are as specified by the Company and the Company reserves the right to change without prior notice.

Conditions of Receiving Health Checkup Service

1. Please do not eat or drink for at least 8-12 hours prior to the checkup (except for dental program and spa). For more information about health checkup preparation, please call your chosen hospital before receiving the service.
2. Present identification card and Smile Healthy Plus checkup voucher to a participating hospital.
3. List of Health Checkup Programs for Pink Gold or Prestige Smile Club Members who are upgraded to this level since 1 January 2014.
