For Thai Nationality

  • Copy of ID card

For Foreigner

  • Copy of Passport with a stamp of Thai Immigration and Valid Work Permit (at least 6 months validity remaining)                              
  • Copy of Marriage Certificated (in case the insured married to Thai)
  • Copy of House Registration (if any)
  • In case for business owner in Thailand, please attach the Commercial Registration Certificated​

Terms and Conditions

  1. Thais and Foreigners
    For Foreigners
    • The foreigners must reside in Thailand at least 1 year and must have a plan to continues staying in Thailand
    • The foreigners works in Thailand, the applicant’s occupation must be specified
    • The foreigners who does not work in Thailand, the applicants must be married to Thai and resides in Thailand more than 1 year
    • The company does not insure foreign tourists
    • The company will adhere to the standard conditions for other conditions apart from aforementioned conditions
  2. Insurable Age 7 – 70 years old
    • Age 7 - 20 years old : 1 Policy per 1 Person
    • Age 21 - 59 years old : Maximum 2 Policies per 1 Person (If the insured wants to get the second policy, the Company will consider it case by case )
    • Age 60 - 70 years old : 1 Policy per 1 Person
  3. The applicants must be healthy and do not have any disabled organ, disability, congenital diseases, severe diseases, Critical illness, Psychotic, Alcoholism and Drug Addict
  4. Weight and Height (BMI) must be in line with the Company’s requirement
  5. The company does not accept high-risk occupations as follows*;
    • Occupations related to boat, pier and water such as fisherman, diver, worker who works underwater, worker whose work related to boat, pier warehouse and labor who loads stuff in and out a boat,worker whose work related to building, adding, repairing or demolishing boat or ship
    • Worker who works in a factory or whose work related to storing, packing or transporting explosives, fireworks, gunpowder, Nitroglycerin especially tanked gas
    • Construction worker whose work related to building or maintaining, dam, bridge or tunnel Kasikornbank Public Company Limited as a broker only
    • Worker who works underground or mine, worker whose work related to demolishing or destroying buildings, glass cleaner whose work related to clean glass of high buildings, worker who installs TV antenna or high voltage, worker who works at a sawmill or whose work related to sawing or cutting lumbers, worker who works at oil rig
    • Professional athlete, a person who joins any kind of sport competition or sport activity such as balloon, glider, bungee jumping , rock climbing, boxer, parachuter
    • Acrobat, stuntman, motorbike taxi rider, service driver, messenger
    • Flight attendant or bus attendant
